LOTO - Lock-Out/Tag-Out. Process of preparing equipment for maintenance.
LTT - Lock/Tag/Try. Another term for preparing equipment for maintenance.

Blind - Piece of flat metal inserted in between two pipes. Described by size and rating. Example: 6" x 150#; 6 inches is the diameter with a pressure rating of 150 pounds per square inch.
Gasket - A material, like rubber, or a part, such as an O-ring, used to make a joint fluid-tight.
Flange - The joint where two pieces of pipe join together.
Danger Tag - All components that are closed, blinded, etc. will have a tag attached to the component to indicate that it is not in its normal position.
Lockout List - A list of all pieces of equipment that will be locked and/or tagged out. This includes but not limited to valves, breakers, local start/stop switches, etc. This is one of the things that our program will print automatically and is a big time saver. It also tells the operator what things need to be under LOTO, thus eliminating the human error of forgetting something.
Blind List - Another list that our program prints out and has the same values of the Lockout List. This list tells the employee where the blinds will be located, what size blind to be used, who installed and removed them, etc.
Equipment History – Another database that our program has to track the history of things that are locked out. You are able to search by equipment number, date ranges, or equipment name.
Other ListsPre Startup Checklists - Things that must be done before a piece of equipment can be started. Examples are frame oil pumps running on compressors or checking the oil level on a piece of equipment.
Critical Alarm Checklist - When pieces of equipment come down for turnarounds, all the alarms on that piece of equipment must be checked to ensure they are functioning correctly.Pre Shutdown Lists - This list can make for successful turnarounds by listing all things that need to be done before the turnaround ever starts. This includes equipment inspection, all lockout lists, blind lists that need to be printed, links to P&ID’s, etc.
Post Shutdown Lists - This list compiles things that did not get done due to time, money, unavailability to get a special valve, etc. and provides a way to keep track of these things for a later date.Blind and Gasket Staging List - This list is for maintenance people and it tells them what size and type of blinds and gaskets are needed. Also it gives detailed descriptions of where these flanges needing the blinds and gaskets are located.
Affected Employee - An employee whose job requires him/her to operate or use a machine or equipment that is being serviced or is in maintenance under lockout or tag out; a person whose job requires him/her to work in an area in which such servicing or maintenance is being performed.
Authorized Employee - A person who locks out or tags out machines or equipment in order to perform servicing or maintenance. An affected employee becomes an authorized employee when that employee's duties include performing servicing or maintenance covered under this section.
Energy Isolating Device - A mechanical device that physically prevents the transmission or release of energy, including but not limited to the following: a manually operated electrical circuit breaker, a disconnect switch, a manually operated switch by which the conductors of a circuit can be disconnected from all ungrounded supply conductors, and in addition no pole can be operated independently, a line valve, a block valve, and any similar device used to block or isolate energy. Push buttons, selector switches, and other control circuit type devices are not energy isolating devices.
Lock out - The placement of a lock out device on an energy isolating device, in accordance with an established procedure, ensuring that the energy isolating device and the equipment being controlled cannot be operated until the lockout device is removed.
Lock out Device - A device that utilizes a positive means such as a lock, either key or combination type, to hold an energy isolating device in the safe position and prevent the energizing of a machine or equipment. Included are blank flanges and bolted slip blinds.
Tag out - The placement of a tag out device on an energy isolating device, in accordance with an established procedure, to indicate that the energy isolating device and the equipment being controlled may not be operated until the tag out device is removed.
Tag out Device - A prominent warning device, such as a tag and a means of attachment, which can be securely fastened to an energy isolating device in accordance with an established procedure, to indicate that the energy isolating device and the equipment being controlled may not be operated until the tag out device is removed.
